Your knee may feel stiff when your feet first touch the floor. The first few steps can ache, then movement may loosen the joint. Stairs, a long drive, or rising from a chair may bring soreness back. Swelling may make the knee feel full or hard to bend. These changes often come from wear in the whole joint, an old injury, or a recent rise in activity. What to notice before you get moving

Notice the exact sore area and the length of your morning stiffness. Notice swelling, warmth, clicking, catching, or any sudden buckling. Think about any fall, twist, or longer walk before the ache began. Your regular doctor or nurse will want to know what helps it ease.

Small details make the exam more useful.

Morning stiffness that eases with motion often goes with arthritis. Pain after one wrong step may come from a strain or torn tissue. Soreness at the front can rise with stairs or standing from a low chair. No single symptom proves the cause. The exam may show one cause, suggest another, or leave some doubt.

What to try while the knee is settling

For several days, do less of whatever made the knee sore, but keep it moving gently. Short, level walks may feel better than one long outing. After activity, a cold pack over thin cloth may ease swelling and aching. Before moving, gentle warmth may make a stiff knee easier to bend. Check with your regular doctor before using medicine for knee soreness.

Aim for calm, steady movement that you can manage again tomorrow.

Use a rail on stairs and avoid deep bends while the knee is sore. Firm, comfortable shoes may reduce strain while you walk on hard ground. If each day feels a little easier, build activity slowly. If it doesn't improve, keeps swelling, or limits sleep, arrange an exam.

The RESTORE trial - a participant-, injector- and assessor-blinded RCT of 288 adults aged 50+ with symptomatic medial knee OA (Kellgren-Lawrence 2-3) - compared three weekly intra-articular PRP injections against saline placebo, with co-primary endpoints of 12-month knee pain and medial tibial cartilage volume on MRI. PRP did not beat placebo on either. It is the single best-designed test of the specific claim that PRP changes joint structure, and it was negative.

  3. A randomized controlled study of 111 patients aged 18-65 with hip osteoarthritis compared three weekly ultrasound-guided injections of PRP, hyaluronic acid, or both. PRP had the lowest VAS at every follow-up and beat HA significantly at 6 months (mean VAS 21 vs 44). But the WOMAC advantage present at 2 and 6 months was NO LONGER significant at 12 months, and the combination arm did worse than PRP alone.

  4. The PRIMA trial randomized 100 patients with ankle osteoarthritis to two ultrasound-guided intra-articular PRP injections or saline placebo. AOFAS scores improved 10 points with PRP (63 to 73) and 11 points with placebo (64 to 75); the adjusted between-group difference over 26 weeks was -1 (95% CI -6 to 3, p=0.56). The authors state the results do not support using PRP for ankle osteoarthritis. The evidence for a biologic in one joint does not transfer to another.
