When a sore knee is worth an exam

Some mornings begin with a stiff knee that eases after breakfast. That kind of soreness can usually wait for your next regular visit. A sudden hot joint, fast swelling, or an injury that stops you walking needs quicker care. The difference rests on what changed, how severe it feels, and whether you are ill. When you aren't sure, call your usual doctor or a nearby care center for advice.

What to carry into your knee visit

Bring any X-ray report and a list of your current medicines. Write down where the knee hurts and which activity causes the soreness. Include any swelling, clicking, catching, or trouble while using the stairs. Write what you already tried and how your knee responded each time.

A few plain notes will help you remember the main points during the visit.

Choose loose clothing so the clinician can see, bend, and examine your knee. A medical provider is part of the licensed clinic staff and will examine your knee before discussing care. The clinician may watch you walk or ask you to rise from a chair. Tell the provider which everyday task you would most like to regain.

When to seek care without delay

Seek urgent help when fever or illness comes with a knee that is hot and very swollen. Seek care after a fall if you cannot bear weight or the knee looks out of place. New calf swelling with shortness of breath also needs urgent attention. Don't wait for a later appointment when any of these symptoms are present.

Get prompt care for sudden leg weakness or numbness that is new.

A knee that locks may need an earlier exam, especially after a twist. Call your own doctor when soreness wakes you often or worsens quickly. These signs don't always mean something serious, but they need proper checking.

What to ask when ordinary care has not helped

Ask which exam findings were firm and which possible causes remain uncertain. Decide whether the main goal is a quieter knee, steadier movement, or both. Ask how the proposed care is done and what recovery may involve. You can also ask what would make another choice more suitable.

The Cochrane review of exercise for knee osteoarthritis found high-quality evidence that land-based therapeutic exercise provides short-term benefit in pain and physical function, sustained for at least 2-6 months after the programme ends, with mild transient soreness the only reported adverse effect across 45 trials. It is the best-evidenced treatment for this condition and it costs nothing per injection.

A four-arm, multicentre, single-blind phase 2/3 randomized trial of 480 knee OA patients (KL II-IV) compared autologous bone marrow aspirate concentrate, autologous adipose stromal vascular fraction and allogeneic umbilical-cord-tissue mesenchymal stromal cells against a corticosteroid injection control. At 12 months NONE of the three orthobiologic injections was superior to another, or to the corticosteroid control, and none of the four groups showed a significant change in MRI osteoarthritis score from baseline. No procedure-related serious adverse events occurred.

  4. A 2026 systematic review and meta-analysis of 28 randomized trials of intra-articular mesenchymal stem cell-based therapies in knee OA found significant improvements in several pain and function measures (delta-VAS MD -1.67; KOOS pain MD 15.37) but NO significant difference in WOMAC, KOOS quality of life or the Lequesne index, and MRI-based WORMS scores were non-significant - indicating no consistent structural benefit. Its own conclusion: these therapies serve a primarily SYMPTOM-modifying rather than STRUCTURE-modifying role, with higher frequencies of local reactions to weigh against the symptomatic benefit.

FDA states plainly that no stem cell, exosome, stromal vascular fraction, umbilical cord blood, Wharton's jelly or amniotic-fluid product has been approved for the treatment of ANY orthopedic condition - it names osteoarthritis, tendonitis, disc disease, tennis elbow, back pain, hip pain, knee pain, neck pain and shoulder pain individually. The only FDA-approved stem cell products in the United States are cord-blood-derived blood-forming stem cells for disorders of the hematopoietic system, and there are currently no FDA-approved exosome products.
